Lee County School Systems Enrollment Requirements

1.WITHDRAWAL PAPERSfrom previous school.

2.BIRTH CERTIFICATE

3.STUDENT’S SOCIAL SECURITY CARD

4.Up-to-date ALABAMA IMMUNIZATION CERTIFICATE.

5.A copy of PARENT/LEGAL GUARDIAN’S DRIVER’S LICENSE, or other picture ID.

6.If the student does not live with parents, or if parents are divorced, then we must have a COURT DECREE declaring guardianship to the person registering the student.

7.PROOFS OF RESIDENCY showing you live in Lee County:
   {MUST PRESENT TWO (2) ITEMS LISTED BELOW}:

* APARTMENT/HOME LEASE AGREEMENT

* HOME PURCHASE AGREEMENT (HOUSE NOTE)

* Utility Bill – WATER, POWER, PHONE OR CABLE

(Cell phone bills are NOT accepted)

* PROPERTY TAX RECORD

* PARENT ID (ONLY IF IT HAS THE CURRENT ADDRESS)

***All information must show the Lee Road address and the Parent/Legal Guardian’s name.

8.If the legal parent or guardian registering the student resides in someone else’s home other than their own, a NOTARIZED RESIDENT AFFIDAVIT must be presented along with two proofs of residency.

9.Any student, who is re-entering after interrupted attendance, shall comply with the foregoing requirements.
